Clinical reasoning — template with patient case and SBAR
Template for a 45-minute clinical reasoning session. Patient case with SBAR report, first impression via word cloud, live differential diagnosis, lab interpretation as a deepening layer, and reflection. With scaffolding for MBO level 4 and HBO. Fill in your own case, keep the didactic structure.
Use this templateThe phases
- 1Case presentation· 5 min
Patient introduction via SBAR. Vital parameters, history, first findings.
- 2First impression· 5 min
Word cloud: which thoughts come to mind? Students see the collective impression — often surprisingly diverse.
- 3Differential diagnosis· 10 min
Multiple choice with the top 3 most likely diagnoses. Discuss the live distribution.
- 4In-depth questions· 10 min
Open question: which information would you want to have? Discuss the top answers as a class.
- 5Lab result (layered)· 5 min
Layered slide: the teacher reveals the result, students interpret it in the context of earlier hypotheses.
- 6Diagnosis + treatment· 8 min
MC: likely diagnosis and first treatment action. Prioritize via the ABCDE system.
- 7Reflection + exit· 2 min
Open question: what did you learn? The cockpit shows understanding per learning objective.

Frequently asked questions
For which healthcare programmes does this template work?+
MBO nursing level 4, HBO Bachelor of Nursing, medicine, allied health programmes (physiotherapy, occupational therapy, speech therapy) and specialist follow-up training. The structure (presentation → first impression → hypothesis → testing → diagnosis) is overarching for clinical reasoning across all healthcare disciplines.
May I use real patient data in a case?+
No. LectaMe is an educational tool, not a healthcare information system. Always use anonymised cases or fictional personas. For healthcare programmes a data processing agreement (DPA) is available that formally records this. AI personas from Image Studio provide a neutral alternative to stock photos of patients.
How do I adapt this template for my field?+
Keep the phase structure (presentation → impression → hypothesis → testing → diagnosis → reflection). Replace the medical content with your field: for physiotherapy "differential diagnosis" can be replaced by "intervention choice", for speech therapy by "treatment plan priorities". The didactic build-up stays the same.
How does scaffolding work in this template?+
Three scaffolding layers are built in. Base layer for everyone: presentation + simple differential diagnosis. Deepening: lab interpretation. Further deepening: complex comorbidities. Students who get stuck on the basics receive extra hints; students who finish quickly open the further layers.
